ABSTRACT

Ureterocele is a cystic dilation of the distal ureter that exits into the urinary bladder. It is a rare condition in adults of which women are the most affected. Its clinical manifestations are very broad and among them the presence of a vulvar mass and symptoms of urinary obstruction stand out. We report the case of a strangulated ureterocele prolapse in a 54-year-old woman with a vulvar mass and signs of urinary obstruction. Initial management with reduction of the mass and finally decided to perform a ureteral reimplantation. Favorable evolution in medical follow-up.

ABSTRACT

Hyperimmunoglobulin E syndrome or Job syndrome is defined as an immune and connective tissue disorder that results in complications such as invasive infections. On the other hand, Fournier gangrene is a rapidly progressive necrotizing fasciitis that affects the perineal area, has a polymicrobial etiology, and its treatment consists of rapid and aggressive surgical debridement, antibiotic therapy, and hemodynamic support. We present the case of Fournier gangrene in an 11-year-old boy with a diagnosis of hyperimmunoglobulinemia E. who was admitted with scrotal necrosis with favorable evolution after initial medical management.

Introducción: El Perú es el segundo país con mayor carga de Tuberculosis y el primero en reportar casos de tuberculosis multidrogorresistente (TB MDR) en Sudamérica. La resistencia a la rifampicina es considerada por la Organización Mundial de Salud como un marcador para tuberculosis multidrogorresistente, ya que diversos estudios han demostrado que más del 90 por ciento de cepas con resistencia a rifampicina en escenarios de alta prevalencia de TB MDR, se comportan como multidrogorresistentes. Por otra parte, la mono resistencia a rifampicina es un fenómeno poco frecuente que no ha sido estudiado ampliamente en nuestro medio. Objetivos: Determinar la proporción de casos con resistencia a rifampicina por prueba de sensibilidad rápida que resultan multidrogorresistente mediante la respectiva prueba de sensibilidad convencional. Asimismo, determinar la proporción de pacientes con mono resistencia a rifampicina y describir su distribución según condición infección por VIH y tratamiento antituberculosis previo. Diseño Se evaluó a los pacientes con tuberculosis pulmonar que cuenten con pruebas de sensibilidad rápida (MODS, GRlESS, Genotype MTBDRplus) con resistencia al menos a rifampicina y cuya cepa haya sido estudiada con una prueba de sensibilidad a drogas de primera línea (proporciones en agar en placa o MGIT) que figuran en la base de datos del Registro Médico Electrónico de la Estrategia Sanitaria Nacional de Prevención y Contra de la Tuberculosis (ESN PCT), durante el año 2013 y que pertenezcan a la jurisdicción Lima y Callao. Resultados: El 93 por ciento de las pruebas con resistencia a rifampicina por prueba de sensibilidad rápida correspondió a cepas con multidrogo resistencia. Asimismo se determinó que el 5 por ciento de la muestra total eran monorresistentes a rifampicina por la prueba de sensibilidad convencional, con una proporción de pacientes con infección por VIH baja y con una mayor frecuencia en aquellos sin tratamiento antituberulosis previo...


Background: Peru is the second country with more burden TB disease and the first in reporting cases with MDR TB in South America. Rifampicin resistant is recommended by WHO as a proxy of MDR TB, because several studies have demonstrated that a high proportion of rifampicin resistant strains are multidrug-resistant. On the other hand, rifampicin mono-resistance is uncommon, which have not studied broadly in our country. Objectives: We determine the proportion of rifampicin resistant tests that are MDR. Furthermore, we determine the proportion of rifampicin mono-resistance and describe the frequency of HIV infection and previous tuberculosis treatment in patients with mono-resistance tests. Design: We evaluated pulmonary TB patients with rapid OST with rifampicin resistant (mono-resistance and MDR) with their respective first line drug susceptibility testing (proportion method or MGIT) which data belong to Electronic Medical Register (EMR) of ESN PCT of time period 2013 and Lima and Callao region. Results: 93 per cent of rifampicin resistance tests were multidrug-resistant. Besides, we found that 5 per cent of total tests were rifampicin mono-resistant. Patient with rifampicin mono-resistance had low proportion of HIV infection and previous TB treatment in our country. Conclusions: Rifampicin resistant is a marker of TB MDR in our country because the high proportion of rifampicin resistant strains that are MDR and low proportion of rifampicin mono-resistance.
